If you change your name, move address or change your contact telephone number, please visit the Health Centre to complete the appropriate form so we can update your records. Alternatively, you can contact reception on 01595 693201 or email the practice at [email protected]
It is important that you inform the practice as soon as possible of any changes to personal details, so your medical records remain accurate and up to date.
If you move outside the Lerwick practice area, you may need to register with a GP surgery closed to your new address. If you are unsure, please contact reception for advice.
